Who does the CARE Team serve?

The CARE Team serves the University Community. Anyone is able to submit a report if they are concerned about the behavior of a student, employee, or anyone else in the University Community (e.g. guests, vendors, affiliates).

How does someone report an individual to the CARE Team?

Anyone is able to submit a report to the CARE Team if they are concerned about the behavior of a student, employee, or anyone else in the University Community (e.g. guests, vendors, affiliates). Reports can be sent directly to a member of the CARE Team, via  utahtech.edu/report-a-concern or click the link in the bottom right corner.

What happens after a report is submitted?

Once a referral is received, the CARE Team will then assess and evaluate the incident(s) and make recommendations to the appropriate University personnel to address the concern if warranted. Potential responses may include no action, monitoring, further investigation, mental health referrals, intervention, identifying appropriate resources, criminal charges, disciplinary action, and outcome follow-up. All available information, evidence, and facts deemed necessary to maintain the safety and security of the University are used.

CARE Team referrals are discussed by team members during weekly meetings throughout the academic year and at other times as needed. This team determines if other University offices may have information to help the team understand the level of concern. The CARE Team decides who the most appropriate person is to reach out and address referred concerns. In some instances, this can be accomplished without mentioning the details of the referral.

What kinds of concerns are appropriate to report?

Any person with a concern about a member of the University Community may report. The following examples are categorized by type, but are not an exhaustive list:

  • Academic: student has excessive absences, missed multiple assignments, and is not responsive to instructor outreach, etc.
  • Campus/Social: student has expressed severe homesickness, reported serious roommate issues, lack in connection, etc.
  • Medical: student has on-going treatment, has left campus for surgery, reported a medical condition of concern, etc.’
  • Mental health: a student or employee has become withdrawn, is exhibiting out of the ordinary behavior, reports experiencing acute anxiety, develops attendance issues, submits an assignment with troubling content, etc.
  • Personal: Student or employee has lost a close friend or family member, reports that they or their parent/family member recently lost employment, or other personal matter leading to distress for the student or employee.

What information should individuals include in the report?

Reports of concerns are reviewed by the University CARE Team. In order to ensure that the concern is dealt with effectively, the team will benefit from the most complete description you can provide: examples of observed behaviors or details of concerning interactions, others who may have witnessed or been affected by the situation, texts of worrisome emails, etc. It is also helpful to know of any steps you have already taken to address your concerns, such as conversations you have had to express your concern with the individual, the individual’s advisor/mentor/supervisor/another staff member.

Is the CARE Team report confidential?

The CARE Team only shares information with those needing to know in order to review, and if appropriate, respond on behalf of the University to the reported concern. It is important to note that confidentiality of the report is not assured and should not be expected because of the complexity in responding to various reports of concern. The individual about whom you report a concern may also realize that you submitted a report through indirect ways (i.e., details related to that incident, information only shared by the individual with one person at the University, etc.).

How do I know if an individual has been reported to the CARE Team?

Only CARE Team members are aware of the names of individuals on the CARE Team reports of concern and are provided the details needed to review and respond to the reported concern. A secure and confidential system is used for receiving the reports of concern.

Will I be notified about any corrective action?

The ability of the CARE Team to provide information to the original reporter is determined by University policies/procedures and the Federal Family Educational Rights and Privacy Act (FERPA).
