REPORTING A CONCERN ABOUT AN INDIVIDUAL TO THE CARE TEAM
To report a concern about a member of the University Community to the CARE Team, please submit a referral through utahtech.edu/report-a-concern and include a brief narrative of the individual’s concerning behavior. If you are concerned about an individual demonstrating any distressing or disruptive behavior, please explain your concern in detail. After submitting a report, a University official will reach out to the person of concern in a timely manner. A crisis or emergency should be reported to the Utah Tech University Police Department by calling 911 (immediate threat/concern). Please call UT Police Dispatch 435-627-4300 for non-emergency/crisis reports.
The CARE Team meets if needed on an emergency basis, as well as on a regular weekly schedule during the academic year (and as needed during the summer) to review reports of concern received. All information received by or related to the activities of the Team is treated as confidential among members of the CARE Team, and it is documented and maintained in a secure and confidential format, protected in accordance with FERPA, University policy, and applicable law and administrative rules.
